Meet and Greet
First Appointment: Meet and Greet
Step One: Bring the following with you:
- ID
- Medical Insurance Cards
- Medication List
- Make time for 30 minutes, including driving time
- List of questions regarding the concierge service
Step Two: Meet the doctor!
During the meet and greet, the patient and the doctor can introduce themselves to each other and better understand how the concierge service can benefit the patient over the traditional primary practice.
Step Three: Sign Up!
- Once the Meet & Greet ends, the patient can decide if concierge medicine is the right fit for them. If so, they can enroll the same day, complete their new patient paperwork, and schedule their next appointment.
- Together, the patient and the provider will determine the best next step: a comprehensive New Patient Visit to take a deeper dive into their health history or proceeding directly to an in-depth annual physical.
- If they need more time to decide, that's perfectly fine. The patient can contact us anytime to enroll, schedule their next appointment, and complete their new patient paperwork when they're ready.
Second Appointment: New Patient Visit
During this visit, the patient and the doctor dive more in-depth into the patient’s health.
We recommend bringing the following: Any medical records the patient has with them
- Any medical records the patient has with them
- A list of medical questions they would like to consult the doctor about
- Make time for 1 hour, including driving time
- Medication List (if it changed between meet and greet to the new patient)
Third Appointment: Annual Physical
Based on the doctor’s recommendations, the next step is typically an annual physical. The annual physical (which you can learn more about here) is split into two different appointments: the Pre-Physical & the Complete Physical Exam.
After completing all pre-physical testing, the patient will return for a comprehensive annual physical exam. During this visit, they will meet with the physician for a thorough examination, review all laboratory and diagnostic test results, and develop a personalized health plan for the year ahead.
